Overview
The CONNECT Management team is in the process of updating the IMS Request Intake form. To gather user feedback to learn and understand how the form and workflow is working for users. The UX Team used Microsoft Forms to create a 7-question survey to identify findings and learnings.

what we learned
Most users typically use the IMS Request form at the beginning of the project.
Participants were unclear on who the point of contact was for person submitting or person on behalf of and were confused for what information goes in the request title and description box.
Participants had challenges with attaching multiple files.
100% of participants found the form helpful.
Users want to know if the request has been submitted and who will be working on it.
Users expect request approval email at the beginning and a request completion email end of the project request.

Form Improvements
Provide form fields with tool tips/help text for users to gain context in order to fill out the request (i.e. examples, WalkMe).
Add drag-and-drop functionality for image uploading.
Add a wysiwyg editor (i.e. bold, bullets, etc.) to specific form fields.
Future Features
Provide a visual indicator to track progress for the specific workflows (i.e. Editorial Review Board).
Provide responsive forms to automatically adjust fields depending on type of request that has been selected.
Pre-populate form fields for return users (data population). 4. Option to save and re-use frequent requests for return users
